To increase efficiency and convenience for new individuals at Invictus Healthcare System, we now offer online new patient registration. This allows us to set the standard of structured and effective care before our patients walk through our doors. This will save time at your first visit and allow us to better understand your needs.
To access the new patient registrations, simply hover over our 'Resources' tab, then click on "New Patient Registration." This will lead you to the first page of our registration. Once you complete the paperwork, hit the "Submit" button to finalize the process.
By filling out the paperwork ahead of time, you will not need to show up 30 minutes early beforehand. Instead, please arrive 10 minutes early to ensure that your appointment with one of our highly-skilled providers starts on time.
All the information submitted in the online form is secure, confidential and protected under HIPAA.
